UNITED TELEPHONE EDUCATIONAL
FOUNDATION SCHOLARSHIP
About this page - This application will allow you to easily
fill out the Scholarship Application utilizing the Internet.
To navigate through the application, use the TAB key or click
on the text boxes.
You have two options when using this
Internet Scholarship Application:
- Use the "Print Application" button to generate
a copy of your completed application suitable for printing.
Once you have done this, you must print the application
using the print feature within your browser and
mail the completed
application to the following address:

P.O. Box 729
Langdon, ND 58249-0729
Please remember that this option only allows you to print
the application and it will not automatically be submitted
to United Telephone.
- Use the "E-mail Application" button
and the application will be submitted to United Telephone
via the
Internet. You
will receive a confirmation within seventy-two hours
from your submission. Please retain a copy of your application
for your records. The scholarship application will not
be considered received until you receive your confirmation.
In the event you don't receive a confirmation, please
resubmit
your application, or e-mail us at utcfound@utma.com.
Due date for returning the application is March 31, 2008.
